You will need Adobe Acrobat Reader which can be downloaded free here: 
Once downloaded, all you need to do is print it off, fill in the Information Sheet, sign one copy of the contract and return them to us along with proof of age and a proof of your addres. Instructions on how to do this can be found on the downloadable document
